UnitedHealth Group is seeking a work-at-home case manager in the Brooklyn, NY area. You will be making outbound calls to assess members’ health status, identifying gaps in treatment plans, providing patient education, coordinating care, and more.
You must have a high school diploma or its equivalent, a current unrestricted RN license in the state, two or more years in a hospital, acute care, or direct care setting, and two or more years as a telephonic and/or field case manager. Additionally, you must have basic Microsoft Word and Windows experience, as well as strong organizational and multitasking abilities.
