UnitedHealth Group is hiring work at home patient care coordinators in the U.S. You must either be in the EST time zone or willing to work EST time zone hours. This is a full-time, home-based role.
Work at home patient care coordinators will work 8-hour shifts Monday to Friday, with occasional overtime.

“The primary role of the Patient Care Coordinator is to support the nursing team with scheduling patient visits for home-based, alternate care settings and ambulatory infusion suites were applicable, and creating/completing clinician visit logs confirming nursing visits for timely billing. Additional responsibilities include triage of patient phone calls, as well as tracking, faxing and timely follow up of physician orders.”

High School Diploma / GED (or higher) or equivalent

Experience with Microsoft office applications – Microsoft Word, Excel, PowerPoint, Outlook (ability to create, edit, save and send documents, spreadsheets, presentations and correscpondence)

Excellent interpersonal, communication and organizational skills /multitasking.

Able to work with a cross-functional team of professionals including dietitians, pharmacists and pharmacy technicians, nurses, case managers, discharge planners, physicians, outside nursing agencies, patients, families and caregivers.

Required to have a dedicated work area established that is separated from other living areas and provides information privacy

Ability to keep all company sensitive documents secure (if applicable)
